words, the sensitivity of the Duke criteria actually appears to be better than that of the Beth Israel criteria modified with the addition of echocardiographic findings. Furthermore, modification of the Beth Israel criteria may result in the use of various unvalidated and possibly incomparable diagnostic classifications. Another point raised by Martos-Perez et a1. is that of the specificity of the Duke criteria. As previously stated [2], I agree that the Duke criteria should not be substituted for the Beth Israel criteria until their specificity has been evaluated. We initiated such an evaluation in a retrospective study of 100 patients with suspected IE, and we found that the specificity of the Duke criteria was 99% (95% CI, 97%-100%) [3]. It would be interesting to evaluate specificity of both the Duke

Lysogeny and bacteriophage host range within the Burkholderia cepacia complex.

The Burkholderia cepacia complex comprises a group of nine closely related species that have emerged as life-threatening pulmonary pathogens in immunocompromised patients, particularly individuals with cystic fibrosis or chronic granulomatous disease. Attempts to explain the genomic plasticity, adaptability and virulence of the complex have paid little attention to bacteriophages, particularly ...

Naturally occurring anti-IFN-gamma autoantibody and severe infections with Mycobacterium cheloneae and Burkholderia cocovenenans.

Recently various genetic defects in immunity mediated by interferon gamma (IFN-gamma) have been described, including mutations in the IFN-gamma receptor 1 (IFN-gammaR1) and receptor 2 (IFN-gammaR2), signal transducer and activator of transcription 1 (STAT 1), and interleukin 12 receptor beta 1 (IL-12Rbeta1), and IL-12 p40 genes. These mutations are associated with the occurrence of severe infec...

A murine model for infection with Burkholderia cepacia with sustained persistence in the spleen.

Burkholderia cepacia is an opportunistic pathogen that causes severe systemic infections in patients with chronic granulomatous disease (CGD) or with cystic fibrosis (CF), but its mechanisms of virulence are poorly understood. We developed a murine model of systemic infection in wild-type (WT) and gamma interferon knockout (GKO) BALB/c mice to facilitate dissection of components of pathogenicit...

Chronic granulomatous disease presenting as refractory pneumonia in late adulthood

We present a case of refractory pneumonia in an adult patient with underlying chronic granulomatous disease (CGD). Her lobectomy tissue grew B urkholderia cepacia and histopathology revealed diffuse severe pneumonic consolidation with suppurative/necrotizing granulomata. An initial attempt to find an underlying immune deficiency was unsuccessful. Following recurrent invasive infections, repeat ...

Microbiological and epidemiological features of clinical respiratory isolates of Burkholderia gladioli.

Burkholderia gladioli, primarily known as a plant pathogen, is involved in human infections, especially in patients with cystic fibrosis (CF). In the present study, the first respiratory isolates recovered from 14 French patients with CF and 4 French patients without CF, identified by 16S rRNA gene analysis, were tested for growth on B. cepacia selective media, for identification by commercial ...
